As featured on the Taber'sPlus DVD... the Nursing Assessment Tool is an adult med-surg assessment form that provides a guide for creating a complete nursing assessment from head to toe.

How to fill out the form:

1. Save the document to your desktop or another folder.
2. Open the document and click in the first field (Name) and begin filling in the document.
3. To navigate through the fields, you can either click on the next field or use the "tab" key to go to the next field.
4. You can print and save the form as a PDF.
5. To print: Click on the "Print Form" key on the first page, or use the "print" icon in the PDF toolbars.
6. To save: Click "File" then "Save As" and rename the document and place it into the appropriate folder.
